OK - well the DOS version of Ghost probably uses the system BIOS for disk access. So on reboot, the BIOS would see a USB drive with one partition.
The 2nd partition is not needed - but some systems will boot as a floppy disk and fail with flashing cursor if it is not present. The 4th ptn is created by E2B when you pick an ISO to run, but it is deleted the next time you load the E2B menu and is type 0 anyway so should be ignored by BIOS and DOS.
When testing BIOS issues like this - ALWAYS switch off the PC - insert USB drive - switch on PC. Do this each time you test booting. This is because, with some BIOSes, they only determine the USB 'type' on a cold boot (power up or hard reset) and not a warm reboot (software reset).
You could convert the ISO to a .imgPTN file and use a file extension of .imgptnAauto and see if that works... then you should only get one partition.
