The first time I made the DLC2016 ISO it did not boot. I then used RMprepUSB - Ctrl+F11 (boot from ISO in QEMU) to test it and it reported a corrupted file. I then made the ISO again and that time it worked. So check your ISO actually works by booting directly from it in a VM.
